First Things First:
Once you are inside the Client Portal, find the folder that is labeled Client Read First. It contains the following documents that you need to see before looking over the other materials in our Portal:

1. Dr John’s Plan Summary– This is the core of our nutrition- program. Review This prior to your first visit with Dr. John. Have questions ready😊!
2. Client Data Sheet– Please fill out and mail or email a copy to Dr John.     See addresses below.
3. Medical History Sheet– Fill out sign and return as noted above.
4. Disclaimer Sheet– Please read, sign, and return as noted above.
5. Others– Other documents in the READ FIRST folder are important!
